And just to confirm, you can reproduce this from a fresh file, with only
the following in the preamble?

  \usepackage{amssymb}
  \DeclareUnicodeCharacter{25A2}{$\square$}

The reason I ask is that I wonder if something else in your main
document is triggering the issue.
