You might want to open a new document, type a formula, and see if 
preview works there. If yes, then the issue is specific to the document 
excerpted above. If preview does not work with a new document, most 
likely either a helper program is missing. In that case you can try the 
following.

  * Close LyX if it is open, then open a command window and start LyX
    with the option "-dbg graphics".
  * Start a new document and add a small math formula, noting beforehand
    what the last message in the command window was. (Everything to that
    point can be ignored.)
  * As new messages show up, look for an error message. If you are not
    sure what you are seeing, copy just the messages from the point
    where you type the math formula and paste them into a message here.
