Adding index to ToC and complied PDF

texindy built the index when run from the command line. In the KOMA-Script
book class document in Settings I set the Index Generator processor to
texindy and the option to idxtotoc. In Settings -> Document Class I set
Class Options -> Custom to idxtotoc as shown in the Wiki.

The index does not appear in the ToC or following the Bibliography. What
have I missed?
