Preamble header issues

You had some problems:

\cleardoublepddpage

it is \cleardoubleoddpage

In the documents preamble (Document->Setting->Preamble)
you also had the wrong

\pagestyle{default}


However,

\usepackage{palatino}

makes no sense, it is superseeded by the following

\usepackage{mathpazo}

attached your now working mwe.
