Display of Cross-References in Outliner

I think some of the changes to tooltips and display of cross-references 
have had some (possibly unintended) side-effects in the outliner. In 
particular, if "Format cross-references in the work area" is unchecked, 
then we often get things like "2.2" or "Section 3.2.4" in the outliner, 
rather than "Ref: LABEL" or "Format: LABEL", which is what we get if it 
is checked. This is, in fact, very inconsistent, and seems to depend 
upon whether the label precedes the reference or the other way around.

The obvious thing to do would to be to go back to displaying "Ref: 
LABEL", or whatever. But I'm wondering whether we can do better. The 
LABEL is going to be the same for every one of the cross-references, 
which are grouped together under the label itself. So that's redundant 
(and makes the displayed string long). One option, then, would just be 
to leave it off and display "Ref" and "PageRef" and so forth. But I was 
also thinking about using what's shown in the dialog:

     <reference>
     (<reference>)

and so forth, or perhaps something similar. Thoughts?
