Maybe I was not precise enough, try this one:
1. go to the document setting -> modules
2. click and hold *on arrow* of 'Accesibility' item (so it gets unrolled)
3. while holding move mouse strictly vertically down to the line of Braille item (but stay horizontally ahead of the item)
4. kaboom


Any better?
I can reproduce on two machines with Qt 5.15.2 & 5.12.8.
