I've done a new pass over MathML and XHTML code to remove a few more HTML
entities that were left (&XXX; where XXX is not a number, amp, lt, gt). I
don't think any others remain, but they might cause similar issues if a
test case starts exercising those code paths (in case it helps for future
failures).
