As long as I have a few cases to work on, that's perfect :)!

I pushed a few commits, they seem to improve the situation. If that's not
enough, I propose to remove the functionality altogether (i.e. stay at the
same level as 2cb7050d010435c1b4345e7924699c6cfb0042da): the behaviour in
DocBook and XHTML is very different regarding sectioning (especially for
the top-level section), I'm less and less confident that a similar
implementation is viable.
