This would be the patch below, but this changes the icons to use the 
outline-inout ones. This is not exactly what we wanted. What would be a 
good icon for the combined action?

The depth-increment/decrement icons would be fine (like libreoffice) 
would be OK, but command-alternative uses the icon of the first element.
One solution is to accept as icon for "command-alternative foo;bar" the 
file command-alternative_foo_bar.svgz (which is not difficult).

PS: this makes me see that I should provide rtl icons for 
outline-in|out, since it contains numbers.
