Cross-Ref Dialog Improvement?

The attached patch, based upon one posted to #11466, adds 'pretty refs'
to the cross-reference dialog. Any thoughts about it? Should this be
made optional or something with a checkbox in the dialog?

Something similar could presumably be done in the TOC.

By the way, this makes UpdateType redundant in many places, possibly
all, so if we go ahead I'll remove all that.
