TOC view for each kind of item in outliner

I have part of a patch that does this for footnotes, specifically, since
they actually get re-numbered per chapter, which makes it VERY
confusing. I think similar things happen with tables, figures, etc,
though that varies by document class, so we need some way to control it
through layout. It was when I figured that out that I realized this was
going to take more time than I had, but I should go back to it....
