[LyX/master] Add three new files from Andrew Parsloe. These do for AMS-type theorem environments what the already existing "by chapter" modules do for non-AMS ones.

I'm looking into some fixes to the theorem style and noticed that there
is a lot of duplication. Is there a reason for this? I'm not very
familiar with our layout mechanism, but why don't we centralize the
common definitions and, if needed, use ModifyStyle?
